ABSTRACT

As a novel star molecule, gasdermin D (GSDMD) plays an important role in the amplification of immune inflammatory response and the process of pyroptosis. After being cleaved and activated by caspase-1, the N-terminal of GSDMD is rapidly released, which anchors on the cell membrane and forms pores, thereby leading to pyroptosis, accompanied by the release of a large amount of the strong proinflammatory factors IL-1β and IL-18. Acute/chronic liver inflammatory response and cell death are the common pathological features of liver diseases such as viral hepatitis, alcoholic liver disease, nonalcoholic fatty liver disease, autoimmune liver disease, liver failure, and hepatocellular carcinoma. This article summarizes the basic structural characteristics of GSDMD and elaborates on its important role in the pathological progression of various liver diseases. In addition, it is proposed that prevention and treatment strategies with GSDMD as a potential therapeutic target can provide new ideas for further studies on the clinical prevention and treatment of liver diseases.

ABSTRACT

Liver failure is a common severe liver disease syndrome in clinical practice and is one of the critical medical conditions in internal medicine. Massive hepatocyte death is the main pathological feature of liver failure, and its core mechanisms include endotoxin, immune response, and inflammatory cascade reaction. Effective regeneration of hepatocytes to compensate liver function is the physiological basis for promoting the good prognosis of liver failure, which directly affects the prognosis and quality of life of patients with liver failure. It has been found in clinical practice that liver failure patients with a low serum level of cholesterol tend to have an extremely high mortality rate, but as an index of hepatocyte anabolism, the association between cholesterol and hepatocyte regeneration has not been taken seriously. Based on the association between cholesterol and liver regeneration, this article reviews its significance and potential value in the clinical treatment of liver failure, in order to understand the pathogenesis of liver failure from another perspective and provide new ideas for the diagnosis and treatment of liver failure and the development of drugs.

ABSTRACT

Objective:To investigate the effects of compression treatment on occurrence of venous thromboembolism (VTE) after tension-free inguinal hernia repair.Methods:The retrospective cohort study was conducted. Of 13 263 patients, 9 995 with compression treatment after tension-free inguinal hernia repair were divided into compression group and 3 268 without compression treatment after tension-free inguinal hernia repair were divided into non-compression group. Observation indicators: (1) compression treatment of patients in the compression group; (2) occurrence of VTE after tension-free inguinal hernia repair in the two groups; (3) analysis of influencing factors for VTE after tension-free inguinal hernia repair. Follow-up using telephone interview was performed to detect history of patient's thrombosis, medical history of patient's family and the incidence of postoperative VTE up to February 2018. Measurement data with skewed distribution were represented as M ( P25, P75) or M (range), and comparison between groups was analyzed using the Wilcoxon rank sum test. Count data were described as absolute numbers or percentages, and comparison between groups was analyzed using the chi-square test. Univariate analysis and multivariate analysis were conducted using the Logistic regression model. Results:(1) Compression treatment of patients in the compression group: of the 9 995 patients in the compression group, 6 086 underwent compression treatment with 0.5 kg of sandbag or 500 mL of packed 0.9% sodium chloride solution, 1 881 underwent compression treatment with trusses, 745 underwent compression treatment with girdles, 675 underwent compression treatment with elastic underwear combined with 0.5 kg of sandbag, and 608 underwent compression treatment with elastic underwear. (2) Occurrence of VTE after tension-free inguinal hernia repair in the two groups: patients of the two groups after matching were followed up. Occurrence of VTE after matching were 15 and 1 in the compression group and non-compression group, respectively, showing no significant difference between the two groups ( χ2=2.010, P>0.05). (3) Analysis of influencing factors for VTE after tension-free inguinal hernia repair: results of univariate analysis showed that cases with varix of lower limb, cases with oral contracep-tives or hormone replacement therapy history, cases with VTE history, clinical classification, clinical typing, surgical method, cases with anticoagulant drugs history, cases undergoing oral antiplatelet drugs, cases undergoing postoperative VTE prevention with medication were related factors for occurrence of VTE after tension-free inguinal hernia repair ( odds ratio=13.98, 37.71, 19.21, 4.43, 4.21, 0.07, 0.08, 0.10, 31.04, 95% confidence interval: 3.15?62.11, 8.35?170.24, 6.15?60.00, 1.43?13.76, 1.20?14.82, 0.01?0.49, 0.02?0.27, 0.04?0.29, 8.53?112.93, P<0.05). Results of multivariate analysis showed that cases with VTE history and surgical method were independent influencing factors for occurrence of VTE after tension-free inguinal hernia repair ( odds ratio=7.78, 11.19, 95% confidence interval: 2.06?29.42, 1.45?86.55, P<0.05). Conclusion:Cases with VTE history and surgical method are independent influencing factors for occurrence of VTE after tension-free inguinal hernia repair.

ABSTRACT

Liver failure is a common critical medical disease, and extensive liver cell necrosis within a short period of time exceeds the regeneration capacity of liver cells and thus results in an extremely high fatality rate. Promotion of effective liver regeneration is the key to antagonizing liver failure. Recent studies have shown that bile acid, farnesoid X receptor (FXR), and intestinal microecology play an important role in liver failure and liver regeneration. This article reviews the association between bile acid, FXR, and intestinal microecology and their role in liver failure and liver regeneration, so as to provide new ideas for the treatment of liver failure in clinical practice.

ABSTRACT

Objective:To investigate the effects of compression treatment on occurrence of seroma after tension-free inguinal hernia repair.Methods:The propensity score matching and retrospective cohort study was conducted. Of the 6 600 patients, 4 939 with compression treatment after tension-free inguinal hernia repair were divided into compression group and 1 661 without compression treatment after tension-free inguinal hernia repair were divided into non-compression group. Observation indicators: (1) the propensity score matching conditions and comparison of general data between the two groups after matching; (2) compression treatment of patients in the compression group after matching; (3) occurrence of seroma after tension-free inguinal hernia repair in the two groups after matching; (4) analysis of influencing factors for seroma after tension-free inguinal hernia repair. Follow-up using outpatient examination was performed at postoperative 14 days and 30 days to detect occurrence of seroma after tension-free inguinal hernia repair of patients up to January 2018. The propensity score matching was realized using the nearest neighbor method with 1∶1 ratio and caliper setting as 0.01. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was analysed using the t test. Measurement data with skewed distribution were represented as M ( P25, P75) or M (range), and comparison between groups was analysed using the Wilcoxon rank sum test. Count data were described as absolute numbers or percentages, and comparison between groups was analysed using the chi-square test. Univariate analysis and multivariate analysis were conducted using the Logistic regression model. Results:(1) The propensity score matching conditions and comparison of general data between the two groups after matching: 3 322 of 6 600 patients had successful matching, including 1 661 in each group. Cases with smoking history, cases with coronary heart disease, cases with diabetes, cases with reducible hernia, irreducible hernia, incarcerated hernia, strangulated hernia (hernia grading), cases with Lichtenstein repair, cases with transabdominal preperitoneal patch repair, totally extraperitoneal repair(surgical methods), operation time, cases with emergency operation before propensity score matching were 1 110, 273, 333, 4 606, 63, 262, 8, 1 636, 2 515, 788, 60 minutes(50 minutes, 90 minutes), 155 in the compression group, and 233, 55, 87, 1 572, 28, 57, 4, 478, 941, 242, 60 minutes(45 minutes, 80 minutes), 29 in the non-compression group, respectively, showing significant differences in the above indicators between the two groups ( χ2=54.713, 12.927, 4.721, 11.218, 16.656, Z=-7.598, χ2=8.891, P<0.05). After propensity score matching, the above indicators were 231, 63, 82, 1 579, 20, 61, 1, 526, 826, 309, 60 minutes(45 minutes, 81 minutes), 34 in the compression group, and 233, 55, 87, 1 572, 28, 57, 4, 478, 941, 242, 60 minutes(45 minutes, 80 minutes), 29 in the non-compression group, respectively, showing no significant difference in the above indicators between the two groups ( χ2=0.018, 0.624, 0.157, 0.159, 0.240, Z=0.001, χ2=0.468, P>0.05). (2) Compression treatment of patients in the compression group after matching: of the 1 661 patients in the compression group after matching, 968 underwent compression treatment with 0.5 kg of sandbag or 500 mL of packed 0.9% sodium chloride solution, 109 underwent compression treatment with trusses, 112 underwent compression treatment with girdles, 311 underwent compression treatment with elastic underwear and 161 underwent compression treatment with elastic underwear combined with 0.5 kg of sandbag. (3) Occurrence of seroma after tension-free inguinal hernia repair in the two groups after matching: patients of the two groups after matching were followed up at postoperative 14 days and 30 days. Occurrence of seroma after matching were 44 and 15 in the compression group and non-compression group, respectively, showing a significant difference between the two groups ( χ2=13.299, P<0.05). (4) Analysis of influencing factors for seroma after tension-free inguinal hernia repair. Results of univariate analysis showed that cases with direct inguinal hernia, cases undergoing compression treatment with 0.5 kg of sandbag or 500 mL of packed 0.9% sodium chloride solution were related factors for occurrence of seroma after tension-free inguinal hernia repair ( odds ratio=0.518, 4.689, 95% confidence interval: 0.271-0.989, 2.730-8.055, P<0.05). Results of multivariate analysis showed that compared with no compression treatment, cases undergoing compression treatment with 0.5kg sandbag or 500 mL pack of 0.9% sodium chloride solution was an independent risk factor for occurrence of seroma after tension-free inguinal hernia repair ( odds ratio=4.698, 95% confidence interval: 2.734-8.073, P<0.05). Conclusion:Compared with no compression treatment, cases undergoing compression treatment with 0.5 kg of sandbag or 500 mL of packed 0.9% sodium chloride solution is an independent risk factor for occurrence of seroma after tension-free inguinal hernia repair.

ABSTRACT

Liver failure is a syndrome of severe liver diseases commonly seen in clinical practice, and it has a high mortality rate and thus becomes one of the critical diseases in internal medicine. Massive hepatocyte death and the extent of hepatocyte death exceeding the liver’s regenerative capacity are considered the core events in the development and progression of liver failure, and direct injury and immune-mediated inflammatory injury are the two main factors in this process. An increasing number of evidence has shown that host immune response and inflammatory cascade play an important role in the pathogenesis of liver failure. This article reviews the mechanism of action of immunoregulation (congenital and adaptive) and inflammatory injury (inflammation inducers, receptor cells, and inflammatory mediators) in the process of hepatic failure, as well as the interactions between immune response and immune cells and between inflammatory response and inflammatory factors, in order to help understand the pathogenesis of liver failure and provide new ideas for the diagnosis and treatment of liver failure and drug research and development.

ABSTRACT

OBJECTIVE:To study the protective effect of tadehaginoside(TA)on liver fibrosis model mice induced by carbon tetrachloride(CCl4),and to investigate its mechanism. METHODS :Kunming mice were randomly divided into normal group , model group ,colchicines group (positive control ,0.2 mg/kg),TA low-dose ,medium-dose and high-dose groups (3,6,12 mg/kg),with 10 mice in each group. Those groups were intraperitoneally injected with 10% CCl4-olive oil solution (5 mL/kg)to induce liver fibrosis model twice a week ,for consecutive 8 weeks;except that ,the normal group was intraperitoneally injected with olive oil. From 3rd week ,the mice in each administration groups were given relevant medicine intragastrically ,normal group and model group were given constant volume 2% sodium carboxymethylcellulose solution intragastrically ,once a day ,for consecutive 6 weeks. The contents of ALT ,AST,HA and IL- 6 in serum of mice were test ed by ELISA. The contents of Hyp , SOD,MDA and GSH-Px in liver tissues were detected by spectrophotometry. mRNA expression of Col- Ⅰ,TIMP-1 and TIMP-2 in liver tissue was detected by RT-PCR. The protein expression of MMP- 2 and TGF-β1 in liver tissue was detected by Western blotting. RESULTS : Compared with normal group,the contents of ALT,AST,HA,IL-6,MDA and Hyp,the mRNA expression of Col- Ⅰ,TIMP-1 and TIMP- 2,as well as the protein exp ression of MMP- 2 and TGF-β1 were increased significantly ,while the contents of SOD and GSH-Px were decreased significantly (P<0.01). Compared with model group,the contents of ALT ,AST,HA,IL-6,MDA and Hyp ,the mRNA expression of Col- Ⅰ,TIMP-1 and TIMP- 2,as well as the protein expression of MMP- 2 and TGF-β1were decreased significantly ,while the contents of SOD and GSH-Px were increased significantly(P<0.05 or P<0.01). CONCLUSIONS :TA has a significant protective effect on liver tissue and anti-fibrosis effects in liver fibrosis model mice ,the mechanism of which may be associated with inhibiting lipid peroxidation and collagen synthesis , down-regulating the mRNA expression of Col- Ⅰ,TIMP-1 and TIMP- 2 as well as the protein expression of MMP- 2 and TGF-β1.

ABSTRACT

Immune cascade due to inflammasome has become a research hotspot in recent years, and the role of NLRP3 inflammasome in the development and progression of liver diseases has attracted more and more attention. This article analyzes the action characteristics and regulatory mechanism of NLRP3 inflammasome in liver diseases, such as viral hepatitis, liver fibrosis, nonalcoholic fatty liver disease, liver failure, and liver cancer, and establishes targeted therapy based on NLRP3 inflammasome to regulate immune response triggered by inflammasome, in order to provide new ideas for the prevention and treatment of liver diseases.

ABSTRACT

Objective: To investigate the effect of icaritin on maturation and mineralization of mouse osteoblast MC3T3-E1 cells and its mechanism. Methods: The cultured MC3T3-E1 cells were divided into blank control group, CXC chemokine receptor type 4 (CXCR4) inhibitor (AMD3100) group, icaritin group, and icaritin plus AMD3100 group. The expression of CXCR4, stromal cell-derived factor 1 (SDF-1) and osteogenesis-related genes and proteins were detected by real-time RT-PCR and Western blotting after drug treatment for 24 h. The alkaline phosphatase (ALP) activity was determined with ALP kit on d3 and d6; calcium nodules were detected by alizarin red staining after drug treatment for 14 d. Results: Real time RT-PCR showed that compared with the blank control group, relative expressions of CXCR4, SDF-1 and osteogenesis-related genes in icaritin group were significantly increased (PPCXCR4 gene was decreased (PPPPPConclusion: Icaritin may promote maturation and mineralization of mouse osteoblast MC3T3-E1 cells through CXCR4/SDF-1 signaling pathway.

ABSTRACT

Purpose To study clinical pathological characteristics,margin status and its influencing factors in different type of facial basal cell carcinoma (BCC).Methods The histopathological features,margin status of the first frozen section and influencing factors was retrospectively analyzed,with review of the relevant literature.The primary outcome variable was the rate of initially positive frozen section margins.Multivariable Logistic regression was used to study histologic subtype,surgical margins,tumor size,location and other factors influence on the rate of initially positive frozen section margins.Results The pathological subtype is the major risk factor,the infiltrative (OR =4.463,95% CI =1.919-10.380,P <0.05) and morpheaform (OR=5.018,95%CI=2.025-16.623,P<0.05) had higher risk on positive surgical margin compared with the nodular.The rate of initially positive frozen section margins of nodular and superficial BCC at different margins were observed but the difference were not significant (P > 0.05).Conclusion The pathological subtype is the major risk factor.Surgical excision with a 3 mm margin can achieve ideal results for nodular and superficial BCC in facial area.

ABSTRACT

OBJECTIVE:To prepare Frovatriptan succinate film-coated tablet,and investigate its in vitro dissolution behavior. METHODS:Using lactose monohydrate,microcrystalline cellulose,dioxide,silica,sodium carboxymethyl starch and magnesium stearate as accessories,Frovatriptan succinate tablet was prepared. Using opadry premix spray-coating liquid,Frovatriptan succinate film-coated tablet was prepared. Single factor test was used,using moisture,angle of repose,rigidity,friability,disintegration time and dissolution rate as indexes,to screen the formulation;using dissolution degree as index,coating material dosage was screened. The dissolution curves in vitro of self-made tablets and imported tablets in water,0.1 mol/L HCL,pH of 5.5,6.8 phos-phate buffer solutions were compared. RESULTS:The optimal formulation of Frovatriptan succinate uncoated tablet was as follow as frovatriptan succinate 3.91 mg,lactose monohydrate 99.18 mg,microcrystalline cellulose 33.06 mg,magnesium stearate 1.40 mg,sodium carboxymethyl starch 1.05 mg,silica 1.40 mg;optimal coating weighed quality was 2.0%-4.0%. In the 4 mediums, the dissolution behavior of self-made tablets and imported tablets were similar. CONCLUSIONS:Frovatriptan succinate film-coated tablet is prepared successfully,and its in vitro dissolution behavior is similar to the imported preparations.

ABSTRACT

Objective:Setting up a queuing simulation model to study the allocation and usage of MRI in certain regional hospital in East China. To find out the causes and put forward suggestions. Methods: Statistical method was used for statistical analysis of MRI inspection time. Queuing simulation model was used to analyzing MRI allocation and used in 7 top hospitals. The waiting queue length, average queue length, sojourn time and waiting time was calculated.Results: The average MRI examination waiting time of the 7 top hospitals in the whole region is 0.403 h. The waiting time of 2 hospitals is more than 40 min while which is less than 20 min in 3 hospitals. The equipment utilization rate is higher in 2 hospitals (vacancy rate is 11.9%-16.4%) while which is lower in 2 other hospitals (vacancy rate is 52.3%-58.9%).Conclusion: The problem of health allocations of resources could be solved by establishing regional MRI examination center radiation regional around.

ABSTRACT

Objective To evaluate preperitoneal tension-free herniorrhaphy for incarcerated and (or) strangulated inguinal hernia.Methods During Mar,2008 to Mar,2015,89 incarcerated and (or)strangulated hernia patients(incarcerated hernia group) and 1 741 primary inguinal hernia patients (elective group) underwent preperitoneal tension-free herniorrhaphy.Results The operation time (42 ± 8 min),length of stay (4.0 ± 2.6 d) and the time return to work (9.0 ± 3.3 d) in incarcerated hernia group were longer than in elective group of (38 ± 4 min),(3.0 ± 0.6 d) and (8.1 ± 2.5 d),respectively (all P <0.01).Blood loss [(10 ± 14 ml) vs (7 ±4 ml)] was compareble,P =0.148.There were 1 infection case and 20 seroma cases (22.5%) vs 5 infection cases and 187 seroma cases (12.7%) all P >0.05.Followup found hernia recurrence in one case in elective group.Conclusion Preperitoneal tension-free herniorrhaphy is safe and effective for the treatment of incarcerated and(or) strangulated hernia.

ABSTRACT

Objective To study the morphological changes of external eyes before and after epicanthoplasty combined with blepharoplasty and to explore a simple and efficient method to correct epicanthus. Methods A total of 120 eyes in 60 patients were operated on with the inverted L-sharp epicanthoplasty combined with double-eyelid blepharoplasty and the follow-up began 2 months after the surgery. The height and physiognomic length of the eyelid and the length and angle of the eyelash were measured and compared with preoperative meansurements, as well as the distance of the inner canthus. The ratios between the inner-canthus distance and the physiognomic eyelid breadth and the diameter of eyebrow and eye were also described. Results The contours of the eyelid fissure and the double-eyelid were natural and the surgical scare was seen unconspicuous 3 months after the operation. Changes in the palpebral fissure , the angel and length of the exposed eyelash were all significantly different by paired-sample t test (P < 0.01). The vertical and horizontal lines through the entocanthion and the vertical line through the gnomic ectocanthion combined with the horizontal line through the brow ridge formed a square structure. Conclusions This inverted L-shape epicanthoplasty combined with blepharoplasty may be effective in reconstructing the contours of the entocanthions as well as the shape of the double eyelids. It can also achieve better cosmetic effects in the proportional relation between the eyes and eyebrows. Therefore, it deserves clinical promotion.

ABSTRACT

Objective To explore the clinical effect of inverted L-shaped incision combined with medial canthal tendon plication for moderate and severe epicanthus correction.Methods Improved method of inverted L-shaped incision combined with medial canthal tendon plication were performed in 42 cases of moderate and severe epicanthus.The epicanthus covered more than 50% lacrimal caruncle with single-fold eyelids or unconspicuous double-fold eyelids.Among them,40 cases received doubleeyelid plasty simultaneously.Results The follow-up time of 38 cases ranged from 6 to 24 months.37 cases received double-eyelid plasty simultaneously.All the patients obtained satisfactory results aesthetically,the contours of two eyelid fissures and the double-eyelid radian looked more natural postoperatively.There was no reoccurrence of epicanthus.Mild scar proliferation was observed in all of the patients in the early 1-2 months of postoperative period and then would fade within three months.Conclusions It is effective to correct epicanthus by improved method of inverted L-shaped incision combined with medial canthal tendon placation.

Objective To develop a practical testing system on internet,to offer a modern pattern of practicing testing for diagnostic imaging and to evaluate the effects.Methods Practical testing system for diagnostic imaging based on PACS was set up and its effect was evaluated by students.Results The developed practical testing system combines question bank,image reading tool and answering interface together and has functions of controlling and managing both the questions and students participating in the examination.In the satisfaction survey of students,practicing testing system of diagnostic imaging got a score of 96.75 (full mark 100) and was highly prized.Conclusions This pattern of online testing offers more imaging information and makes practicing examination of diagnostic imaging more vivid and scientific.

ABSTRACT

This study is to investigate the effect of osthol on osteoclasts' activity, bone resorption as well as apoptosis in vitro, and explore the mechanism of osthol in preventing osteoporosis. Osteoclasts were separated from long-limb bones of new born rabbits, cultured in 24-well plate with glass slices and bone slices, and treated by 1 x 10(-5) mol x L(-1) osthol. Osteoclasts were identified by observing live cells with phase contrast microscope, HE staining, TRAP staining and toluidine blue staining of bone resorption pits. The numbers of bone resorption pits were counted as well as the surface area of bone resorption on bone slice. Osteoclasts were stained with acridine orange to detect the cell apoptosis. The ratio of apoptotic osteoclasts was observed under fluorescence microscope. The gene expression of RANKL, OPG, TRAP and p-JNK1/2 protein expression were examined using real time PCR and Western blotting, respectively. Comparing with the control group without osthol, the rates of apoptotic osteoclasts increased obviously and the number and area of bone resorption pits decreased evidently with 1 x 10(-5) mol x L(-1) osthol. There is significant difference between control group and experiment group treated by 1 x 10(-5) mol x L(-1) osthol. Therefore, the osthol through RANK+RANKL/TRAF6/Mkk/JNK signal pathway inhibits the osteoclasts activity, enhances osteoclasts apoptotic and inhibits the bone resorption.

ABSTRACT

Objective To evaluate medical chemistry adhesive in tension-free herniorrhaphy for inguinal hernia. Methods In this study, 100 patients with primary unilateral inguinal hernia were assigned to study group ( n = 50) and control group ( n = 50) during Jun. 2009 and Dec. 2009. Medical chemistry adhesive (n-butyl-2-cyanoacrylate, NBCA) was used in Lichtenstein tension-free hernia repair in study group and suture procedure was used in control group. Patient demographics, operation time,postoperative length of stay, visual analogue scale ( VAS ) score 24 hours after surgery, incidence of postoperative chronic pain and hematoma, recurrence rate, and other complications were compared between the two groups. Results The duration of follow-up ranged from 12 months to 18 months. There were no recurrences or wound infection in the two groups. In study group, no patient complained of chronic pain postoperatively, whereas in the control group, 6 patients ( 12% ) had a significant chronic pain. In study group, 2 patients (4%) had local hematoma after operation, whereas there were 8 ( 16% ) in the control group ( P < 0. 05 ). There were no significant differences between the 2 groups in postoperative length of stay ( P > 0. 05 ), but the operation time and postoperative VAS score in study group ( 38 ± 5 min and 2. 5 ± 0. 6)were lower than in the control group (42 ± 5 min and 2. 8 ± 0. 8 ), ( P < 0. 05 ). Conclusions Application of medical chemistry adhesive in tension-free herniorrhaphy for inguinal hernia is associated with less postoperative pain, lower incidence of hematoma, less postoperative chronic pain and shorter operation time.

ABSTRACT

Objective To explore a new surgical treatment for severe inverted nipple and to reduce the possibility of recurrence. Methods Two opposite long six-side polygon de-epithelialized areas were designed within the range of areola on both sides of the nipple. The four sides in lateral portion were dissected and two wedge-shaped dermal and glandular tissue flaps were created, completely releasing the contractive fiber causing nipple retraction while reserving breast ducts. The flaps were reversed 180 degrees through the tunnel bilateral sides of breast ducts to support the nipple. It was tracted for 3-4 weeks by series 7 needle penetrating the nipple basis. Results All the 14 patients were followed up for 3-18 months, and they were satisfied with the appearance and the aesthesia of the nipple and no recurrence was observed, breast-feed function reserved in 2 cases after childbirth. Conclusions This operation is easy to perform with satisfying aesthetic outcome and breast-feed function remaining.It is an ideal method for correcting serious inverted nipple.

ABSTRACT

BACKGROUND: Bone marrow mesenchymal stem cells (MSCs) have multi-differentiation potential. In the acute wound, MSCs have been demonstrated to have the potential for differentiating into skin cells. However, there are few reports regarding its differentiation in diabetic wound.OBJECTIVE: To observe the feasibility of differentiation of MSCs into the cells of skin appendages under the microenvironment of diabetic wound.METHODS: MSCs were isolated from the bone marrow of rats, purified and cultured. Third or fourth passage MSCs were selected and labeled with 5-bromodeoxyuridine (5-BrdU). The rats were injected intraperitoneally with single administration of streptozocin to establish diabetes model. After 2 weeks, a round skin wound was made on the dorsal back of rats. BrdU-labeled MSCs at a density of 1×10~9/L were injected into the wound of the rats. The specimens were harvested from the wound tissues to prepare sections at 2 and 3 weeks after transplantation, followed by immunohistochemical staining with BrdU or keratin. RESULTS AND CONCLUSION: BrdU positive cells aggregated in the epidermis, dermis and hypodermia. Some positive cells appeared in the sebaceous glands and sebaceous duct cells and expressed keratin simultaneously in serial sections. During diabetic wound healing, MSCs have the potential to differentiate into the cells of the skin appendages.
